Update: Human error led to accident on board of the LPG carrier

The accident on the board of Varun Shipping's LPG carrier ship, that cause the death of 5 people, may have occurred due to a possible human error, a senior company official stated yesterday, citing initial investigations.
After the discharge of liquefied petroleum gas, an elaborate exercise is carried out to clean out residuals of the cargo for which there is a conventional operating procedure, the official informed.
"They 1st remove the residues with inert petroleum gases, after that airing is carried out. The crew members most likely removed the spectacle blind flange partition without realising that the inert petroleum gases are yet to be flushed out."
He told these are not the final findings but evidence gathered till now pointed towards this. "There is a conventional operating procedure that is in place for carrying out such jobs...they seem to have gone wrong in the sequencing as the inert petroleum gases were still present."
